Request a Proposal

To take advantage of all that CareFirst Administrators has to offer, you and your clients should submit the following information in a request for proposal.

  • Current plan effective date and proposed plan effective date
  • Account information: SIC code/nature of industry, number of years in business, address of headquarters and for each location.
  • Census data for each participant: name, date of birth, sex, dependent status and plan option if multiple options available
  • Enrollment counts by month for last 36 months
  • Other plans, if offered
  • Claims experience, including paid claims information by month for the last 36 months
  • Large or ongoing claim information for claims at 50% of the specific for the last 36 months. There are conditions that may not have reach 50% but are ongoing that are of serious nature and are required to be disclosed.
  • Carrier history: Show carriers for the past 5 years and give the prior and renewal rates, and factors and details of current coverage, if self-funded.
  • Specific level, contract basis, coverage under the Spec and Agg and any special contract provisions
  • Plan: copy of the current plan of benefits
