CareFirst Administrators and all of its business associates conduct business in compliance with the standards established by the Health Insurance Portability and Accountability Act (HIPAA). These standards are designed to ensure your privacy and the security of your Protected Health Information (PHI).
The attached forms allow members to:
- Give permission to CFA to release information at your request
- Designate a Personal Representative to act on your behalf
- Revoke permission for an individual to act on your behalf
